What to Bring to Your First Appointment

Please plan to arrive about 15 minutes early, so our front desk team has enough time to assist with any remaining paperwork. To help our medical team understand your health history, please bring:

A list of your current prescription and over-the-counter medications, including dosage and frequency

Medical history information

Details about your previous medical conditions, procedures, and surgeries
